In short
For the hkk health insurance card you need a recent, frontal photo with your face clearly visible against a neutral background. A biometric passport photo is not required. hkk contacts you when it needs a photo – from the age of 15. You then upload it through hkk’s photo upload or in the hkk Service-App, or stick a print onto the reply form from the letter.

Photo requirements
What hkk itself says:
- Portrait 35 × 45 mm without border
- File format JPG, TIF, PNG or BMP, at most 2 MB
- Recent, you are clearly recognisable
- Neutral plain background, face centred, looking straight ahead
- Only you in the picture; glasses only with clearly visible eyes, no tinted lenses
- Religious head covering allowed if the face stays recognisable; not required to be biometric
Photo rejected – what now?
If your photo is rejected, hkk usually tells you why. Common reasons are shadows, a busy background, a tilted head or a face that is too small.
- Take a new photo in daylight from the front, head straight
- Face large enough in the frame (about 70–80% of the image height)
- Have the background replaced – FotoKlar does this automatically
- If the insurer rejects the purchased photo for not meeting its requirements, we refund the €7.99 in full
